Please fill out these forms prior to your first consultation to streamline your visit.
What to expect during your first appointment.
Masks are optional - although we do maintain a sterile office.
Be prepared to fill our some forms about your previous medical and dental history (you can get a head start by printing and filling out the list of forms in the previous section.)
Be aware of our COVID-19 Guidelines. Please continue to maintain a social distance of 6 feet between before and after your appointment.
Get a full, extensive first appointment with Dr. Wong or Dr. Wu.

Transfer history
If you are transferring to our practice from another dental practice, feel free to ask them for your dental history and send them to dentist@gw2dental.com. Don’t worry - we will still do the full checkup on our own.
If you are transferring from our practice to another dental practice, email dentist@gw2dental.com to grab your dental history with us and share with your new dentists. We are sad to see you go :(
